What is Obstructive Sleep Apnea?  什么是阻塞性呼吸暫停的睡眠?

Obstructive Sleep Apnea is stoppages in breathing during sleep due to obstruction of the upper airway. The obstruction of the airway can be due to a blocked now, thick palate, big tonsils/adenoids, and/or a big tongue.



What is Snoring?  什么打鼾

Snoring is due to the vibration of the tissues in the mouth and oral cavity (namely the soft palate, uvula, base of tongue and other soft tissues). Snoring implies an increased resistance to the inflow of air during breathing at the level of the upper airways. Snoring may arise from sinus problems, palate thickness and a big tongue.



What is Snoring / Sleep Apnea Surgery?  什麼是打鼾/睡眠呼吸暫停手術?

These comprise of surgery to the nose, palate and/or tongue.



Nose  鼻子

The nose is physiologically essential in breathing, any swelling within the nose needs to be corrected in order for normal breathing to be restored.


Nose surgery also helps reduce snoring and obstructive sleep apnea to a good extent (especially if done together with other procedures, like palate or tongue surgery).


Type of nose surgery depends on the pathology of the nose, in general, a crooked nose septum will be corrected and enlarged sinus turbinates can be reduced.


Different methods are available (not all are mentioned here). One example would be the radiofrequency of the inferior sinus turbinates (this procedure is simple, minimal pain, quick and has minimal side-effects).



Palate  上颚

Palate surgery is essential to reduce snoring and enlarge the oral airway (this may include a tonsillectomy if the tonsils are enlarged). Approximately 70% of most snoring arise from the palate. Surgery to the palate will reduce snoring and obstructive sleep apnea.


There are different types of palate surgery available (variations depend on the palate anatomy) (not all are mentioned here).


In general, reconstruction of the palate is needed to expand and enlarge the oral cavity airway. The Pang’s Expansion Pharyngoplasty has been shown to improve snoring and sleep apnea in over 80% of patients.



Tongue  舌頭

Patients with big tongues and small oral cavities will inevitably have narrow airways that obstruct during their sleep leading to obstructive sleep apnea.


These patients might need to have some form of tongue volume reduction (e.g. radiofrequency reduction) or tongue suspension suture (to support the tongue and prevent it from falling backwards during sleep).


There are different types of tongue surgery (not all are mentioned here).
